(ACW) - No cost to facility

This is vital for the financial success of the facility for the following reasons:

01One of the biggest reasons the residents are moving out of the facility is because of overall shortcoming in their approach to wellness.

02On the other hand, a properly implemented wellness program can increase each one of your resident’s life on an average by 6 months, resulting in roughly an additional 1.5 million dollars in revenue for every 50 residents.

03Implementing ACW also results in increased operational efficiency.

Benefits and services

Care coordination of the residents between the provider and the POA at least once a month and update the care team.

Monthly meeting on line - update and address the issues with DON.

Provide multispecialty clinic as per the need of the facilities.

Assigned care coach to coordinate care among the attending, the facility, pharmacy, home health agency and Family. Also provide a monthly Pain assessment, Fall risk assessment, Depression score, Hospitalization risk & Mortality assessment.

Automated care -coordination and wellness management (ACM)

Why is it important?

Studies show workforce efficiency in general results in around 80% of the time spent on busy work and 20% of the time spent on productive work. This is where issues arise. The automated care coordination (ACM) takes away the clerical work of wellness coordinators and gives them time to be efficient on matters that matter, and also have more hands-on time with residents rather than being on the phone and faxing. We estimate ACM could increase efficiency of the staff by 30% minimum. Also, with its easy access to providers with in your network, ACM allows tremendous stress reduction.
